Job summary

Herewith is seeking a compassionate Care Partner to support in-home dialysis for a patient transitioning from clinic to home care. This is a one-on-one, consistent assignment with training provided to ensure safe, patient-centered care.

You will assist with setup, monitor treatment, and provide emotional support while maintaining a calm, professional presence. Strong Spanish skills and collaboration with a clinical team are essential.

Qualifications

• Minimum 1 year of dialysis experience (home or clinic).
• Fluent in Spanish (required).
• Open to in-home dialysis care and completing provided training.
• Calm, patient, and dependable during care transitions.
• Strong communication and collaboration with clinical team.

Responsibilities

• Set up in-home dialysis treatment using NxStage or similar systems.
• Maintain a clean, safe treatment environment.
• Keep patient on schedule with dialysis treatments.
• Provide companionship and emotional support.
• Assist with light ADLs as needed.
• Document shift notes and report concerns.
