MDH Network, Inc. is seeking a compassionate Community Activities Specialist to support individuals enrolled in the In-Home Day Program in Burbank, CA. The role emphasizes person-centered, community-based support for individuals with intellectual and developmental disabilities.

As part of the Community Integration Program, you will work 8:00 a.m. to 2:00 p.m. on a Monday–Friday schedule, providing structure, social skills development, budgeting, self-advocacy, and community access support in a

Competenze

• Requires high school diploma or GED.
• Must be 21 years of age or older.
• 2+ years of full-time experience supporting individuals with intellectual and/or developmental disabilities.
• 1+ year of experience supporting individuals with emotional and behavioral needs.
• Ability to read, write, and speak English; bilingual (Spanish/English) preferred.

Mansioni

• Provide structured, individualized support to individuals per IPP/ISP.
• Support community integration, skill development, and independence.
• Ensure compliance with HCBS, Title 17, and Title 22 regulations.
