Job summary

You will connect with clients, develop personalized support plans, and help promote independence, dignity, and well-being through compassionate care and collaboration with supervisors and team members.

Responsibilities

• Assisting clients with daily living activities, such as personal care, meal preparation, and household tasks.
• Providing companionship and emotional support to clients, reducing feelings of isolation.
• Facilitating clients' access to community resources, appointments, and social activities.
• Developing and implementing individualized support plans in consultation with clients and supervisors.
• Monitoring clients' well-being and reporting changes to appropriate personnel.
• Adhering to safety protocols to ensure client and personal safety.
• Maintaining accurate and confidential records of client interactions and progress.
• Transporting clients to appointments or community outings as needed.
• Encouraging client independence and self-advocacy.
• Participating in team meetings and ongoing training sessions.
