Elara Caring in Michigan is seeking a Certified Nurse Aide to join our field-based care team. You will provide hands-on personal care under the supervision of a Registered Nurse, assist with daily activities, and help patients maintain comfort in their homes.

You will monitor vital signs, support safe transfers, follow individualized care plans, and document care while protecting patient privacy. A valid CNA or Home Health Aide certification and reliable transportation are required to serve

Qualifications

• Current State Home Health Aide or CNA certification
• Ability to frequently lift, push, pull, and support up to 50 pounds
• Valid driver's license or reliable transportation to travel between patient homes

Responsabilités

• Provide hands-on personal care under the direction of a Registered Nurse
• Assist patients with bathing, dressing, grooming, toileting, and daily activities
• Take and record vital signs (temperature, pulse, respirations, BP)
• Help patients safely walk, transfer, and use mobility equipment
• Follow patient care plans and maintain documentation while protecting privacy
• Report changes in patient condition to the care team
• Maintain a clean, safe home environment and comply with infection control guidelines
