Signify Health, part of CVS Health, seeks a Full Time Clinician (NP/PA) to perform in-home health evaluations for health plan members. Clinicians will travel across licensed states, using an iPad and a streamlined workflow to complete assessments.

Active licenses and board certification are required, with ability to obtain multi-state licenses and cover counties within the state. This role emphasizes building relationships and delivering personalized care at patients' homes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses as required.
• Local counties within state and flex to other areas of business need.

Responsibilities

• Provide in-home health evaluations to health plan members.
• Travel up to 100% across defined coverage area; may require overnight stays.
• Review medication history and perform physical evaluations during visits.
• Use iPad and clinical workflow; do not prescribe or alter current regimens unless directed.
