Qualifications

• Active, unrestricted NP/PA license in the governing coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Perform in-home health evaluations using iPad and clinical workflow.
• Review medications and medical history; conduct physical examination.
• Coordinate care and connect patients to appropriate services.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide diagnostic and preventive services as indicated.
