Job summary

Signify Health, part of CVS Health®, seeks part-time Per Diem Clinicians (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. Clinicians will visit members at their homes, review medical history, perform physicals, and coordinate with health plans.

Travel within licensed states is required; virtual visits are possible. The role emphasizes compassionate care, accurate documentation, and delivering preventive services without prescribing or ordering labs.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations to plan members.
• Engage with patients at the homes of health plan members.
• Travel locally within licensed states as needed; conduct virtual visits when required.
• Use iPad and clinical workflow; no prescribing or lab orders required.
• Deliver Signify Health services including diagnostics and preventive exams.
