Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, visiting patients in the comfort of their own homes.

The 45-minute visits include medication and medical history reviews, a physical evaluation, and may include optional diagnostic tests. You’ll use an iPad and a simple clinical workflow; no prescribing or lab orders are required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Visit patients in their homes for health evaluations.
• Perform 45-minute visits including history review and physical exam.
• Coordinate diagnostic tests as ordered by health plan when needed.
