Job summary

McGregor PACE, a community-based Program of All-inclusive Care for the Elderly, is seeking a Clinic LPN to join our Cleveland team. The LPN delivers direct nursing care to participants and supports day-to-day clinic operations under RN supervision.

Responsibilities include administering medications, wound care, and assisting in 24-hour care coordination with the multidisciplinary team. Minimum qualifications: Ohio LPN license, pharmacology certification (CLPN), at least three years of medication

Qualifications

• Licensed Practical Nurse (LPN) in the state of Ohio.
• Pharmacology certification (CLPN) is required.
• Minimum three years of medication administration experience.
• Current Ohio driver’s license and reliable transportation.

Responsibilities

• Administer medications to participants within LPN scope of practice.
• Prepare medications for participant homegoing and use in accordance with Ohio law.
• Assist in 24-hour care coordination and multidisciplinary team communication.
• Support data collection and participate in performance improvement initiatives.
