BoldAge PACE is seeking a Licensed Practical Nurse to provide direct nursing care in a PACE clinic, administer medications, and support care plans under supervision. The role emphasizes communicating with primary caregivers and the interdisciplinary team to ensure high-quality, person-centered care for older adults.

Responsibilities include medication administration, patient education, record maintenance, and triage of participant needs.

Qualifications

• Must be bilingual in English and Hmong.
• Graduate of an accredited school of nursing.
• Current LPN/LVN license in good standing to practice in the required state.
• BLS certification within 90 days of employment.
• IV Certification preferred.
• 1 year of experience working with a frail or elderly population preferred.

Responsabilités

• Administers medications and implements primary care orders.
• Evaluates participant complaints and communicates with caregivers and interdisciplinary team.
• Maintains participants’ medical records and safety compliance.
• Triage and respond to participant needs, including emergencies.
• Educates participants and families about medications and treatment instructions.
• Assists with scheduling of appointments and coordination of care as needed.
