Great Lakes Community Action Partnership (GLCAP) is seeking a Home-Based Coach to deliver adult education and family engagement services for Head Start programs. This full-time, 34-hour workweek includes comprehensive benefits and a strong emphasis on health, nutrition, and school readiness.

The role involves coordinating health and developmental services, visiting families, and documenting activities in agency databases to support children’s growth and family needs.

Qualifications

• Associate degree in social services, human services, family services, counseling, or related field.

Responsibilities

• Recruit, enroll and maintain required caseloads according to the ERSEA plan.
• Collate information for health packets and ensure children are up to date with health services according to EPSDT.
• Visit families as assigned and ensure completion of PFCE/education standards; follow up on medical and social service needs.
