Job summary

Children’s Home Society of South Dakota is seeking a Case Manager to provide in-home case management services to children and families in alignment with our trauma-informed mission. You will perform home visits, assessments, referrals, and coordination with community partners while maintaining confidentiality and promoting equity and inclusion.

Minimum two years of experience with children and families; Bachelor’s degree required, Master’s preferred.

Qualifications

• Bachelor’s Degree required, preferably with emphasis in Social Work or related field.
• Master’s degree in social work or similar field is preferred.
• Must be willing to obtain ongoing training as required through the agency and specific to the role.
• Minimum two years’ experience working with children and families.
• Experience in crisis stabilization for children, youth, and families.

Responsibilities

• Provide case management services to children and youth, to include home visits, community navigation, family contacts/visitation, referral for services within the community, transportation, and other needs as identified through case planning process.
• Complete evidenced based assessments during case plan evaluation process for child and/or caregiver.
• Must be able to provide courteous and respectful personal attention to promote individual client satisfaction and the public perception and reputation of the Agency.
• Respond effectively, tactfully, and with maturity in highly-emotionally- charged situations.
• Must be able to conduct oneself in a manner that promotes professionalism and teamwork throughout the Agency, setting an example for others to follow.
