Job summary

Children’s Home Society of South Dakota is seeking a Case Manager to provide in-home case management services and appropriate interventions to children and families. You will work within a trauma-informed framework to foster equity and inclusion.

Qualifications

• Minimum two years’ experience working with children and families.
• Knowledge of crisis stabilization for children, youth, and families.
• Must have CHW Certificate, or ability to obtain certificate.

Responsibilities

• Provide in-home case management services to children and families.
• Complete evidenced based assessments during case plan evaluation for child or caregiver.
• Maintain records per agency policies and confidentiality standards.
• Collaborate with treatment team regarding client and family outcomes.

Bachelor's Degree in Social Work or related field
