Job summary

SYNERGY HomeCare Northeast Metro is seeking compassionate Care Professionals to support clients in their own homes across the Northeast Metro. This part-time role offers flexible scheduling and the potential for full-time hours, with opportunities to earn additional pay through shift differentials and overtime as available.

You will follow each client’s Plan of Care, help with daily living activities, provide mobility support, and offer companionship while maintaining dignity and independence

Qualifications

• 18 years of age or older
• Valid driver’s license with proof of auto insurance and reliable transportation
• Ability to safely perform physical requirements up to 25 pounds
• In-home care experience preferred, but not required

Responsibilities

• Provide personalized, in-home care based on each client’s Plan of Care
• Assist with activities of daily living (bathing, dressing, grooming, toileting, feeding)
• Provide mobility assistance and range-of-motion activities
• Offer companionship and positive social interaction
• Assist with hands-on care as permitted and remind about medications
• Observe changes in condition and report safety concerns
• Document care provided and services rendered accurately
• Maintain a clean, safe, and comfortable client environment
• Collaborate professionally with clients, families, supervisors, and fellow caregivers
• Maintain client dignity, confidentiality, and appropriate professional boundaries

About the Role

SYNERGY HomeCare Northeast Metro is looking for compassionate, dependable Care Professionals to provide quality in‑home care to clients throughout the Northeast Metro.

You’ll provide personalized support based on each client’s individualized Plan of Care, helping them remain safe, comfortable, and as independent as possible in their own homes.

What You’ll Do

Your responsibilities may include:

• Providing compassionate, respectful care based on each client’s individualized Plan of Care
• Assisting with activities of daily living, including bathing, dressing, grooming, toileting, feeding, oral hygiene, and bedtime care
• Providing mobility assistance, including ambulation, exercise, and range-of-motion activities
• Providing companionship and positive social interaction
• Assisting with permitted hands‑on care such as support stockings, ace bandages, and hot/cold therapies
• Providing medication reminders only—Care Professionals do not administer medications
• Observing clients and reporting changes in condition, safety concerns, incidents, or accidents
• Accurately documenting care provided and services rendered
• Maintaining a clean, safe, and comfortable client environment
• Working professionally with clients, families, supervisors, and fellow Care Professionals
• Maintaining client dignity, confidentiality, and appropriate professional boundaries

• Prior in‑home care experience is preferred, but not required
