Job summary

SYNERGY HomeCare Northeast Metro in Andover, MN is seeking compassionate Care Professionals to provide in-home care to clients in the Northeast Metro. You’ll work part-time with flexible scheduling and have the potential to grow hours as available.

You’ll follow individualized care plans, assist with activities of daily living, provide mobility support, and offer companionship. No medication administration, but reminders and careful documentation are required.

Qualifications

• Must be 18 years of age or older.
• Valid driver’s license with proof of auto insurance and reliable transportation.
• Ability to safely perform physical tasks incl. standing, walking, bending, reaching, and lifting up to 25 pounds.
• In-home care experience preferred but not required.

Responsibilities

• Provide compassionate, respectful care based on each client’s Plan of Care.
• Assist with activities of daily living (bathing, dressing, grooming, toileting, feeding, oral hygiene, bedtime care).
• Provide mobility assistance and range-of-motion activities.
• Offer companionship and positive social interaction.
• Assist with approved hands-on care (support stockings, ace bandages, hot/cold therapies).
• Give medication reminders only—no administration.
• Observe changes, report safety concerns, incidents, or accidents.
• Document care provided and services rendered.
• Maintain a clean, safe client environment.
• Collaborate professionally with clients, families, supervisors, and other Care Professionals.
• Maintain client dignity, confidentiality, and professional boundaries.

About the Role

SYNERGY HomeCare Northeast Metro is looking for compassionate, dependable Care Professionals to provide quality in-home care to clients throughout the Northeast Metro.

You’ll provide personalized support based on each client’s individualized Plan of Care, helping them remain safe, comfortable, and as independent as possible in their own homes.

What You’ll Do

Your responsibilities may include:

• Providing compassionate, respectful care based on each client’s individualized Plan of Care

• Assisting with activities of daily living, including bathing, dressing, grooming, toileting, feeding, oral hygiene, and bedtime care

• Providing mobility assistance, including ambulation, exercise, and range-of-motion activities

• Providing companionship and positive social interaction

• Assisting with permitted hands-on care such as support stockings, ace bandages, and hot/cold therapies

• Providing medication reminders only—Care Professionals do not administer medications

• Observing clients and reporting changes in condition, safety concerns, incidents, or accidents

• Accurately documenting care provided and services rendered

• Maintaining a clean, safe, and comfortable client environment

• Working professionally with clients, families, supervisors, and fellow Care Professionals

• Maintaining client dignity, confidentiality, and appropriate professional boundaries
