Apex Companion Care, part of LHC Group, is hiring PRN Home Caregivers in Jacksonville and surrounding areas. You will provide in-home support, personal care, and help with daily living activities to help clients remain in their own homes.

Responsibilities include assisting with hygiene, meal prep, errands, medication reminders, and documenting client conditions using EVV; a valid driver’s license and CPR certification are required or obtainable within 90 days.

Qualifications

• Must have a valid driver’s license and access to a dependable vehicle or reliable public transportation.
• Ability to work flexible hours as required to meet clients’ needs.
• Ability to function in any home situation regardless of client background.
• Current CPR certification or ability to complete within 90 days of hire.

Responsabilités

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
