Job summary

Sharecare, Inc. through CareLinx is actively seeking experienced in-home caregivers to provide non-clinical support to health plan members and private clients. You control your availability via the CareLinx app, and the team matches you with clients that fit your schedule and goals.

On onboarding, you will work with the CarePartner team to find the right matches based on location, needs, and availability. Successful caregivers build consistent schedules, stay engaged via the app, and communicate

Qualifications

Minimum 6 months caregiving experience or relevant education/certification.
Smartphone required to run the CareLinx app for scheduling and documentation.
Ability to communicate in English.
Reliable transportation to client homes.
Must pass a background check.
Program-specific training may be required before shifts.

Responsibilities

Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming, and light housekeeping.
Complete and document all care tasks in the CareLinx mobile app during each shift.
Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team.
Submit timesheets on time through the CareLinx app.
Communicate with your CarePartner regarding any case updates or client changes.
Maintain patient privacy, dignity, and well-being in a safe environment.
Support additional shift opportunities such as transportation, errands, or hospital-discharge escort.
Telehealth navigation—assist clients in connecting with virtual doctor visits.
