Responsibilities

• Daily Living Support (ADLs & IADLs): Assist individuals with meal preparation, personal care, home cleanliness, money management, shopping, and community integration.
• Advocacy & Empowerment: Act as a dedicated advocate, ensuring individuals served are treated with dignity, respect, and independence at all times.
• Documentation & Reporting: Accurately complete charting, logs, and support plan documentation in a timely manner while maintaining HIPAA confidentiality.
• Behavior & Health Support: Implement behavior management strategies according to support plans, perform medically oriented tasks as trained, and remain alert to health/behavior changes.
