Suncrest Companion Services, part of LHC Group, is hiring PRN Home-Based Caregivers to support clients in Folkston, GA and surrounding areas. You will assist with personal care, daily living activities, and accompany clients to appointments, while documenting care and maintaining safety.

Qualifications

• Must have a valid driver’s license, auto insurance, and reliable transportation or access to public transit.

• 6+ months of home care experience preferred.
• Ability to work flexible hours.

As a home-based Caregiver, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home. We are hiring PRN Caregivers for multiple shifts in Folkston and the surrounding areas.

Primary Responsibilities:

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs

Required Qualifications:

• Current driver’s license, vehicle insurance, and reliable transportation or access to public transit

Preferred Qualifications:

• Current CPR/AED certification or ability to complete within 90 days of hire
• 6+ months of home care experience
• Ability to work flexible hours

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $14.00 to $20.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
