Job summary

Suncrest Companion Services, a part of LHC Group, is seeking PRN In Home Caregivers for Folkston, GA. You will provide personal care and assist with daily living activities to help clients stay in their own homes.

Responsibilities include monitoring health, documenting conditions, and assisting with errands while ensuring safety and confidentiality. A valid driver’s license and reliable transportation are required, with CPR/AED preferred or obtainable within 90 days.

Qualifications

• Current driver’s license, vehicle insurance, and reliable transportation.
• Preferred CPR/AED certification if not current, ability to obtain within 90 days.
• 6+ months of home care experience or similar caregiving background.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe home environment and respond promptly to client needs.

As a home-based Caregiver, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home. We are hiring PRN Caregivers for multiple shifts in Folkston and the surrounding areas.

Primary Responsibilities:

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders


• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed


• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system


• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs

Required Qualifications:

• Current driver’s license, vehicle insurance, and reliable transportation or access to public transit

Preferred Qualifications:

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $14.00 to $20.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
