Charis Hearts, Inc. is seeking compassionate caregivers to provide non-medical in-home and community support under the TennCare CHOICES program. Duties include wellness checks, client assistance with admissions, and training fellow caregivers.

Flexible scheduling, top pay, health benefits, and fast onboarding are available. You will work with clients across Tennessee, maintain confidentiality, and document care accurately while adhering to service plans.

Qualifications

• Must be at least 18 years old.
• Valid State ID/Driver's License.
• Strong communication skills, both oral and written.
• Reliable transportation to and from client locations.
• Compassionate and caring nature with a genuine desire to help others.
• Honesty and integrity in all interactions.
• Willingness to attend training sessions provided by referring agencies and MHDD.

Responsabilités

• Provide non-medical in-home and community care per Client Service Plan.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
• Provide non-medical support to agency service recipients.
• Be familiar with and follow all rules, policies, and procedures related to service delivery.
• Submit all required documentation accurately and on time.
• Adhere to the individual service plan for each recipient.
• Maintain confidentiality of all client records and information.
