Job summary

Charis Hearts Inc. is seeking compassionate caregivers to provide non-medical in-home and community care under the TennCare CHOICES program in Tennessee.

You will follow an individualized service plan, perform wellness checks, assist with ADLs, and ensure accurate documentation while maintaining confidentiality. Training is provided, and benefits include health coverage and weekly direct deposit. Flexible scheduling supports work-life balance.

Qualifications

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.
• Strong communication skills – both oral and written.
• Reliable transportation to and from client locations.
• Compassionate and caring nature with a genuine desire to help others.
• Honesty and integrity in all interactions.
• Willingness to attend training sessions provided by referring agencies and/or the Tennessee Department of Mental Health & Developmental Disabilities (MHDD).

Responsibilities

• Provide non-medical in-home and community care per the Client Service Plan.
• Perform client wellness checks to ensure well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on ADLs in the client's home.
• Submit required documentation accurately and on time.
• Adhere to the individual service plan for each recipient.
• Maintain confidentiality of all client records and information unless authorized by written consent.
• Immediately report any incidents of injury involving the service recipient or yourself to your supervisor.
• Report any suspected abuse or neglect to the appropriate authorities and your supervisor.
