Job summary

Elevate is seeking compassionate in-home caregivers to support clients in their own homes in Fort Collins, Colorado. Caregivers will provide nonmedical assistance per care plans, including personal care, meal prep, light housekeeping, and companionship while ensuring client safety and comfort.

Responsibilities include following care plans, reporting observations to supervisors, and maintaining patient confidentiality and professional conduct at all times.

Qualifications

• Must be at least 18 years old.
• Must be able to provide services according to the care plan.
• Excellent customer service and listening skills.
• Excellent oral and written communication skills; able to effectively communicate with the client and the office staff.
• Ability to read and write clear English.
• Able to pass background checks
• Able to pass drug screening
• Current driver's license
• Valid I-9 Employment Eligibility Verification documents
• Valid vehicle insurance coverage and registration

Responsibilities

• Provide in-home care per the care plan and client’s needs.
• Assist with Ambulation, Bathing, Personal Hygiene and Dressing.
• Assist with meal planning, preparation, and light housekeeping.
• Provide companionship and emotional support; document daily activities.
