Qualifications

• Performs personal care activities that assist the patient with activities of daily living.
• Provides hospice supportive care as taught by a nurse.
• Assists with prescribed range of motion exercises and health monitoring.

Responsibilities

• Assist with personal hygiene and daily living activities.
• Escort patient to appointments and run errands as needed.
• Perform light housekeeping and meal preparation per instructions.
• Provide medication reminders and assist with health monitoring.
• Support patients with mobility and transfers and use lifting aids as required.
