Department Overview:
Community Living Connections, Inc. (CLC) is a local, non-profit agency founded in 2004. CLC supports individuals with intellectual/developmental disabilities, physical disabilities, and frail elders to live as independently as possible, in their own homes. The supports CLC provides are flexible, person-centered and committed. Our organization provides support that enables people to live in their own homes and participate in community life. We believe that individuals, regardless of ability, deserve equal opportunities to create their "best life"; they just need additional support in order to do so. Community Living Connections provides in-home staff to help with planning, personal cares, medical support, finances, relationship building, etc. We work in partnership with people throughout their lifetime.

Remote Work Eligibility Detail:
This position is in-person working directly with other people. During COVID-19, all employees must comply with appropriate precautions to ensure the safety and wellness of people we support and coworkers. Employees must maintain 6 ft social distancing when possible. Employees must wear facial masks while working in the homes of people we support, and in the presence of anyone (regardless of indoors or outside). Facial masks and hand sanitizer are provided to employees.

Varies per household. Schedule is determined at the interview, based on your availability. Nights and weekends are a high need.

Position Summary/Job Duties:
Supported Living Staff provide necessary supports in daily living to people who have developmental disabilities. These supports are person-centered and may include assistance with household upkeep, personal cares and wellness, finances, and community integration. Major responsibilities include, but are not limited to, the implementation and monitoring of specific goals as reflected in each consumer's support plan. Supported Living Staff may be reassigned on an as-needed basis to a different site should program needs require it. Responsibilities include:
