Job summary

Geisinger LIFE Geisinger seeks a home care aide to work in participants’ homes, supporting care plans to maximize independence. You will provide direct participant care, including personal care, nutrition and hydration, obtain vital signs, assist with ambulation and transfers, and ROM, and help with medication prompting.

You will also perform light housekeeping, laundry, and supply delivery, participate in team meetings, complete documentation, and maintain safety and respectful communication

Qualifications

• Provide direct participant care including personal care, nutrition and hydration, obtain vital signs, assist with ambulation and transfers, and active and passive range of motion, and assist with prompting for medication compliance.

Responsibilities

• Light housekeeping and assist with laundry; use supplies accurately and economically; deliver medication and supplies as needed.
• May require on-call coverage for after-hours care for emergencies in home.
• Maintain safety with gait belt, transfers, and body mechanics; ensure participant safety and dignity.
• Complete accurate documentation for services provided; communicate effectively with participant and team.
• Attend interdisciplinary team meetings and complete required trainings.
