ExcelaCare Home Health in Roswell, NM is seeking a Registered Nurse PRN to administer skilled nursing care in patients’ homes, coordinate with a multi-disciplinary team, and support families.

The RN will conduct assessments, develop and implement care plans, document status, supervise aides, and participate in on-call rotations. Requires valid NM RN license, CPR, and one year of nursing experience; travel is required.

Competenze

• Graduate of an accredited school of nursing.
• Current State RN license; CPR.
• Driver’s license and proof of auto liability.
• One year of nursing experience.

Mansioni

• Administers skilled nursing care in patients' homes per policy.
• Coordinates care with multi-disciplinary team, patient, family and referring agency.
• Documents patient status, nursing interventions and education; manages Plan of Care.
• Develops and implements a written Plan of Care with physician orders.
• Supervises LVN/LPN and home health aide; participates in case conferences.
• Makes visits as scheduled and participates in on‑call rotation.
• Performs related duties and ensures compliance with Medicare/Medicaid and insurance standards.
