Qualifications

• Graduate of an accredited school of nursing.

Responsibilities

• Coordinates care with multidisciplinary team, patient, family, and referring agency.
• Documents patient status, interventions, and education in timely manner.
• Supervises LVN/LPN and home health aide as assigned.
• Makes visits as scheduled and participates in on‑call rotation.
