Job summary

Liberty Health in Dunn, North Carolina is seeking an experienced Home Care Registered Nurse (RN) to join the Liberty Cares team on a full-time basis. You will provide professional nursing services according to the plan of care and actively participate in interdisciplinary care planning to support recovery at home.

The ideal candidate holds an approved nursing degree, NC RN license, at least one year of clinical experience in acute or long-term care (home health preferred), CPR certification, and

Qualifications

• Must have nursing degree from an approved school.
• Current RN licensure in North Carolina.
• At least one year of clinical nursing experience; home health experience preferred.
• Knowledge of fundamental medical-surgical nursing principles.
• Strong communication skills, written and oral.
• Ability to use a computer and basic software.
• CPR certification.
• Team-based approach to patient care planning.

Responsibilities

• Actively participates in the interdisciplinary care planning process.
• Provides professional nursing services per the plan of care established by the Patient Care Coordinator.
• Promotes favorable outcomes through collaborative practice and appropriate use of resources.
• Documents accurately and concisely to support care delivery and reimbursement.
• Demonstrates positive interpersonal relationships.
• Demonstrates accountability for completing work and professional growth.
