Qualifications

• Four-year college degree required.
• Ability to work with individuals from diverse backgrounds.
• Access to facilities and homes to meet clients.
• Valid Driver's License with clean MVR.

Responsibilities

• Administer state-required assessments (UAI, CARE, MFEI as applicable).
• Develop and implement care plans, monitor and evaluate services.
• Contact each client monthly by face-to-face or telephone.
• Provide information, referrals and resources for service options.
• Coordinate client services with multiple agencies/providers.
• Advocate for the needs of the aging population.
• Assist with other agency programs as needed.
