Job summary

A Path of Care Preferred Pathways in Norman, OK is seeking a full-time ADvantage Case Manager to serve the Southwest Tulsa area. You’ll perform initial assessments, build care plans, and monitor progress, coordinating in-home services to keep members safe and independent in their homes.

Requires Oklahoma RN or LPN license, a Bachelor’s degree with one year in aging or disabled populations, CPR within 60 days, a valid driver’s license, reliable transportation, and willingness to travel;

Qualifications

• Oklahoma RN or LPN license required.
• Bachelor's degree and one year paid professional experience with aging or disabled population.
• CPR certification within 60 days of hire (for RN/LPN only).
• Valid driver's license and a good driving record. Reliable transportation and proof of insurance.
• Ability to travel long distances.
• Experience with the Advantage Program is preferred.

Responsibilities

• Perform initial assessments of member needs, develop plans of care, and monthly monitoring.
• Coordinate services to keep members safe and independent in their home.
• Participate in in-service programs and Advantage training.
