Responsibilities

• Travel to patient homes and manage care plans per physician instructions.
• Provide transportation to clients when needed.
• Assess and document patient condition at each visit.
• Review medications and vital signs; complete evaluation tasks.
• Dress or redress wounds and monitor healing progress.
• Educate caregivers and family on aftercare and ongoing care.
• Maintain patient comfort with palliative care principles.
• Suggest tools or devices to improve quality of life for the patient.
• Insulin or medication management as needed.
