Job summary

Solutions for Care is seeking a Care Coordinator to manage caseloads and conduct in-home assessments in the North Riverside, IL area. You will assess eligibility for state programs, plan care, and link clients to services with documentation in CMIS, PSS, and eCCPIS.

On-the-job training is provided and weekend rotations may apply. The role requires strong organizational and communication skills, a valid driver’s license, and the ability to work independently in a home-visits setting.

Qualifications

• Bi-lingual Spanish/English preferred.
• BA/BS in Social Science, Social Work, or related field, or an RN/BSN.
• One year of program experience in elder services may replace education up to four years.

Responsibilities

• Develop and maintain in-home assessments and individualized care plans.
• Assess eligibility for Illinois CCP and coordinate services.
• Complete required paperwork and maintain client files timely.
• Navigate CMIS, PSS, and eCCPIS for documentation.
• Coordinate referrals to community agencies and monitor progress.
• Communicate with clients, families, and providers clearly and professionally.
• Attend staff meetings and trainings; ensure compliance with program policies.
