The role requires driving, Medicaid billing ability, and strong communication. Competitive pay ranges from $19.00 to $22.26 per hour with comprehensive benefits and field travel.

Qualifications

• Broad knowledge of community service systems.
• Advanced communication skills, both oral and written.
• Advanced organizational skills.
• Ability to bill Medicaid without restrictions.
• Valid driver's license and ability to operate a vehicle.
• Adequate auto insurance coverage.
• Ability to use personal or company vehicle for services.
• Maintain confidential information.
• Ability to meet deadlines in a fast-paced environment.
• Proficient in Internet-based office environments.

Responsibilities

• Engage individuals and families in home and community to identify goals.
• Provide skill building, coaching, and practical supports.

• Assist navigating community resources; collaborate with partners.

• Maintain engagement with clients and caregivers.
• Document in EHR; follow HIPAA and policy guidelines.
• Participate in team meetings and required trainings.
• Travel within designated service areas.
• Develop housing resource guides with partners.
