Adapt of Missouri, a subsidiary of Compass Health Network, is seeking a Community Support Specialist - Residential in St. Louis for a full-time role supporting adults in behavioral health and recovery goals.

Services are delivered in clients' homes, in the office, and out in the community. You will help clients set goals, build daily living skills, and access services such as housing, healthcare, and community resources while collaborating with an integrated care team.

Qualifications

• High School/GED required.
• Associate’s or Bachelor’s degree in a human service-related field preferred.

Responsabilités

• Partner with clients and families using a motivational, strengths-based approach to support progress toward recovery and resiliency goals.
• Provide individualized, holistic services addressing all dimensions of wellness, including physical, mental, and social health.
• Assist clients in developing and achieving goals outlined in their Wellness Plans, including accessing needed services such as healthcare, housing, financial support, and community resources.
• Offer education and skill-building to promote independence in daily living, symptom management, and recovery.
• Collaborate closely with integrated health team members and community partners to ensure coordinated, culturally responsive care.
• Support clients through transitions such as hospitalization and discharge planning, maintaining regular communication to promote continuity of care.
• Complete timely documentation and uphold service and quality standards in alignment with agency guidelines.
• Provide transportation and other direct supports as clinically appropriate. Driving is an essential function of this position.
