Job summary

Compass Health Network is seeking a Community Support Specialist for the Adapt program in St. Louis.

The role focuses on community-based support for adults with behavioral health and substance use recovery goals, meeting clients where they are—at home, in the office, or in the community. You will develop goals in Wellness Plans, assist with accessing healthcare and housing, and work with an integrated care team to promote continuity of care.

Qualifications

• High School/GED required.
• Associate’s or Bachelor’s degree in a human service-related field preferred.
• Must have valid driver's license and agency minimum automobile coverage.

Responsibilities

• Partner with clients and families using a motivational, strengths-based approach toward recovery goals.
• Provide holistic services addressing physical, mental, and social health.
• Help clients develop Wellness Plans and access healthcare, housing, financial support, and community resources.
• Offer education and skill-building to promote independence in daily living and symptom management.
• Collaborate with integrated health team members and community partners to ensure coordinated care.
• Support transitions such as hospitalization and discharge planning with ongoing communication.
• Complete timely documentation and maintain agency guidelines.
• Provide transportation and direct supports; driving is essential.
