Job summary

Children's Home Society of Florida (CHS) is seeking a Behavioral Health Case Manager to assess client and family needs and coordinate services in home and community settings.

You will connect families with resources, advocate across schools and healthcare systems, and document progress in the EHR. The role requires a valid Florida driver's license, a Bachelor's in Human Services, and AHCA Targeted Case Management certification within three months.

Qualifications

• Bachelor's Degree Required in Human Services/Social Work or related degree.
• Minimum 1 year working with severely emotionally disturbed children (or 3 years if degree is outside of Human Services)
• Florida Driver's License with access to a reliable, insured vehicle
• Can work Mon-Fri, 8-5pm, with flexibility to work outside standard hours when needed
• Completion of AHCA-approved Targeted Case Management certification training within three months of hire

Responsibilities

• Conduct comprehensive assessments to identify client and family needs and develop individualized service plans that drive meaningful outcomes
• Connect children and families to critical community resources and treatment services
• Advocate for clients across schools, healthcare systems, behavioral health providers, and community organizations
• Provide in-home support and guidance to families
• Document progress and outcomes through our electronic health record system
• Participate in family service planning, case reviews, and community outreach efforts
