Job summary

PruittHealth Home First Yancey Burnsville is seeking an In-Home Aide III to provide nursing support under supervision, supervise a program aide, and ensure high-quality care for elderly participants with cognitive needs.

Primary duties include monitoring vital signs, assisting with ADLs, and ensuring adherence to care plans. Strong CNA certification and experience in geriatric settings are required; travel is not typically indicated.

Qualifications

• Certified CNA with at least 1 year experience in geriatric care in acute, long term, or home care setting.
• Must be licensed by a physician within 45 days of employment and in good health.
• Freedom from illegal drug use and workplace substance abuse; ineligible if history of abuse.

Responsibilities

• Supervise program aide assisting with nurse care.
• Observe participants and report changes in condition to manager.
• Monitor and record vital signs and weights.
• Provide restorative or rehabilitative nursing services including bowel/bladder management and therapies.
• Assist with ADLs and personal care; manage supplies.
• Apply infection control techniques and ensure skin care.
• Prepare daily diet orders and direct meal service as prescribed.
• Assist in emergency care and monitor dietary/ fluid intake adequacy.
• Coordinate scheduled activities for participants.

Description

JOB PURPOSE:

Nurse Aid - Responsible for providing nursing care as directed by the manager and for supervising the program aid assigned to assist with nursing services.

KEY RESPONSIBILITIES:

• Supervise the program aide assisting with nurse care
• Understand the needs of the elderly and, in particular, the needs of those affected by the cognitive impairments.
• Be sensitive to the needs of family caregivers.
• Be available to work unscheduled days and hours.
• Make a commitment to cooperative performance to attain care, organization and programmatic objectives that may entail performing duties not included in the job description.
• Observation of each participant and immediately reporting any changes in their condition to the manager.
• Monitoring and recording routine vital signs and weights.
• Provision of restorative or rehabilitative nursing service including bowel and bladder management and follow through of physical, occupational and/or speech therapies.
• Provision of direction and assistance to program aides assisting with personal care, incontinency care and maintenance of supplies related to same.
• Provision of prophylactic skin care to avoid breakdown
• Provision of nail care.
• Use of infection control/prevention techniques in the rendering of care.
• Preparing daily diet order.
• Direct day meal services to ensure participants receive meals prescribes by their attending physician.
• Report inadequacies in dietary or fluid intake to the manager.
• Assist in the provision of emergency care.
• Check participant clothing, incontinency and personal care supplies and obtain same to ensure adequacy of amounts on hand.
• Implement and monitor scheduled activities for program participants.
• Assisting patients with ADLs (i.e. bathing, personal hygiene, grooming, dressing, toileting, ambulation, eating)

Qualifications

LICENSURE, CERTIFICATION, AND EDUCATION REQUIREMENTS:

• Certified by a licensed physician within 45 days of employment to be in good health including freedom from communicable diseases, including tuberculosis in the communicable form, according to acceptable screen methods of the Department of HRS.
• Freedom from illegal drug use, and freedom from use and effects of use of drugs and alcohols in the workplace. Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for the position.
• Must be a Certified CNA with at least 1 year experience in the geriatric care in acute, long term, or home care setting.
