Job summary

A leading multispecialty provider is seeking an Advanced Practice Provider (NP/PA) to deliver in-home care for patients with chronic conditions in Iowa. This role focuses on enhancing patient relationships, performing assessments, and educating patients about their conditions. Candidates must have an active RN and NP/PA license, relevant certifications, and 2 years of direct care experience. Qualifications

• Minimum 2 years of direct patient care experience.
• Current and unrestricted DEA certificate, or eligibility to obtain.
• Knowledge of federal/state regulations (HIPAA, OSHA).

Responsibilities

• Provide patient-centered care directly in patients’ homes.
• Perform comprehensive assessments and follow-ups.
• Educate patients on disease management and preventive care.
