Community Health Resources seeks an Outreach Therapist to deliver in-home adolescent therapy through the Multi-Systemic Therapy (MST) program in Norwich, CT or Middletown, CT. You will assess teens, develop individualized treatment plans, and coordinate with families and providers to improve outcomes.

This full-time position involves travel, a flexible M-F 8-8 schedule with some evenings, ongoing supervision, and opportunities for professional growth within a supportive team.

Qualifications

• Master’s degree or higher in a related field.
• Experience with trauma, mental illness and/or substance use disorders preferred.
• LMSW, LPC-A, LMFT-A required. LCSW, LMFT, LPC, Licensed Psychologist preferred.
• Bilingual in Spanish and English preferred.

Responsibilities

• Provides assessment and treatment interventions consistent with the MST model.
• Conducts clinical and risk assessments.
• Collaborates with individuals to create individualized, person-centered treatment plans.
• Maintains client and staff records according to agency policy.
• Provides coverage for team members during periods of absence.
• Participates in weekly supervision, consultations, and meetings.
