Advocate Health in Park Ridge, IL, is seeking a Registered Nurse for our Hospital at Home Program. You will deliver hospital-level care in patients’ homes within the Lutheran General Hospital service area, supporting a combination of acute care tasks and remote monitoring tools.

The role requires an IL RN license, BLS certification, and 2+ years of acute care or ED experience. Travel is local, with a manageable caseload and a flexible schedule including alternating weekends.

Qualifications

• Active RN license in the state of Illinois (IL)
• BLS certification (AHA)
• 2+ years of recent acute inpatient care or ED experience preferred
• Strong IV, phlebotomy, and clinical assessment skills
• Ability to work independently in home settings
• Excellent communication and critical thinking
• Comfortable with technology and remote monitoring tools

Responsabilités

• Deliver acute, hospital-level care in patients’ homes
• Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)
• Monitor patient conditions and recognize changes requiring escalation
• Collaborate with a virtual and in-person interdisciplinary team
• Use evidence-based practice to ensure safe, patient-centered care
• Coordinate discharge planning and ongoing care needs
• Promote a culture of safety and high-quality outcomes
