Job summary

Innovative Hematology, Inc. (IHI) seeks an experienced thrombosis RN for comprehensive anticoagulation care coordination across inpatient and outpatient settings in Indiana. You will focus on patient safety, follow-up, education, and perioperative planning, working with multidisciplinary teams.

The role emphasizes timely patient management, use of dashboards (Power BI), and accurate documentation in the EMR. Minimum 2 years of RN experience and Indiana license required.

Responsibilities

• Coordinate anticoagulation and thrombosis care across inpatient and outpatient settings.

• Assist in perioperative anticoagulation planning and patient education.
• Review hospital notifications and perform timely follow-up and re-education as needed.
• Document anticoagulation events using standardized workflows.
• Support transition of care and referrals to outside providers when needed.
