Job summary

Mental Health Cooperative is seeking a Substance Use Services Intake Clinician to perform comprehensive substance use and behavioral health assessments, determine level of care, and coordinate enrollment. This onsite role emphasizes collaboration with internal teams and external providers to connect clients with appropriate services.

Ideal candidates hold a Master’s in a behavioral health field and Tennessee licensure eligibility (LCSW, LMSW, LPC, MHSP, or LMFT).

Qualifications

• Master’s degree in Social Work, Counseling, or related behavioral health field.
• Tennessee licensure or eligibility (LCSW/LMSW/LPC/MHSP/LMFT).
• Experience conducting comprehensive behavioral health assessments.
• Ability to work onsite as required.

Responsibilities

• Conduct comprehensive substance use and behavioral health assessments.
• Determine appropriate level of care and make recommendations.
• Complete required assessments, consents, screenings, and enrollment documentation.
• Coordinate referrals and enrollments with internal and external providers.
• Provide education about available services and treatment options.
