Job summary

The Grove at Valhalla Rehabilitation and Nursing Center is seeking a Speech Language Pathologist (SLP) to provide and direct Speech Therapy services for patients, including assessment, treatment planning, and coordination with medical staff.

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals.
• Conducts screenings of patients to determine need for intervention.
• Evaluates patients and develops treatment plans and goals.
• Communicates evaluation findings to patients, family and team.
• Develops treatment plans and coordinates with patient and family.
• Monitors progress and adjusts treatment per physician orders.
• Instructs on augmentative communication devices to patients and families.
• Documents assessment, treatment and discharge information.
• Supervises students and volunteers as required.
