Qualifications

Responsibilities

• Develops and implements treatment programs and in-services under physician orders.
• Conducts screenings and evaluations to determine intervention needs.
• Communicates findings to patients, families, and team members.
• Establishes patient schedules and coordinates with the care team.
• Monitors progress and modifications to achieve goals as ordered.
• Recommends equipment and trains patient, family, staff in use of devices.
• Participates in discharge planning and home/community programming.
• Documents results and maintains records, including time tracking.
• Supervises students/volunteers and participates in continuing education.
