Job summary

Responsibilities

• Responds to urgent psychosocial crises in clinical, community, and emergent settings.
• Provides clinical case management addressing medical, behavioral, social and environmental determinants of health.
• Provides consultation to medical, behavioral health, and administrative staff regarding psychosocial factors affecting patient care.
