Job summary

Vernon County is seeking a Social Worker in Training for the Children’s Long-Term Support Unit. The role involves evaluating services for youth under Wisconsin CLTS and CCOP, with independence in navigating state guidelines.

Essential duties include intake, eligibility determinations, service plan development, and ongoing case management for families. Requires a bachelor’s degree in a human service field and a current or soon-to-be social work license.

Qualifications

• Bachelor's degree in a human service discipline is required.
• Must have current Social Work certification meeting Wis. Statute 457 OR ability to obtain within two years.

Responsibilities

• Perform intake and assessments for CLTS and Children's Community Options Program.
• Determine eligibility for CLTS and CCOP.
• Coordinate enrollment and develop, implement, and review service plans.
• Provide ongoing monitoring and case management to recipients and families.
• Serve as liaison between contracted agencies and Vernon County.
• Coordinate and oversee program advisory committee and annual plan.

Education

Bachelor's degree in human service discipline
