Job summary

Livingston County is seeking a Social Work Assistant to provide social work services to patients in a skilled nursing facility. The role operates under NYS regulations with supervision from a master’s level social worker.

You will document patient social histories, assist with discharge planning, and coordinate with families and other agencies. Qualified candidates may enter via one of three options: a BS in Social Work, an AS in Human Services with relevant experience, or a high school diploma

Qualifications

• Bachelor’s degree in Social Work from NYS or regionally accredited college.
• Two years of full-time, paid experience in social work for the Associate’s option.
• Four years of full-time, paid experience in social work for the High School option.

Responsibilities

• Provide social work services to patients, families and responsible parties.
• Coordinate discharge planning and transfers; assist with transportation arrangements.
• Maintain confidential records and progress notes; liaise with care teams and supervisors.
• Aids in arranging personal care services and in-service training on patient rights and psycho-social needs.
