Job summary

Apple Rehab Inc. is seeking a Social Work Assistant to implement and document plans of care for residents’ social and emotional well-being in the EMR.

The role collaborates with nursing, MDS, Admissions and rehabilitation to facilitate discharge plans and educate residents, families, and staff on rights and available services. Qualified candidates will have a Bachelor’s Degree in Human Services or related field, with preferred Social Work background.

• Bachelor’s Degree in Human Services or related field (required).
• Preferred: Bachelor’s Degree in Social Work.
• Demonstrates cooperation and teamwork with staff and residents.

Responsibilities

• Craft and document personalized care plans for residents' social and emotional well-being.
• Collaborate with nursing, MDS, Admissions and rehab to ensure discharge plans.
• Inform residents and families of rights and services and educate staff accordingly.
• Maintain updated resources for residents and families and track concerns.
• Provide timely referrals to community resources and agencies as needed.
• Ensure accessibility and support for residents, families and staff.
