Job summary

A leading substance use disorder treatment center located in Los Angeles seeks a full-time Social Services Care Coordinator. This role involves facilitating and coordinating ancillary needs for clients in treatment, ensuring timely medical documentation, applying for client benefits, and arranging necessary services. Ideal candidates will have strong organizational and communication skills, and be willing to support clients on their path to recovery. Qualifications

• Must be registered and/or certified with state-approved addiction counselor certification entities.
• Must possess a valid California driver's license.
• Willingness to undergo random drug screenings.

Responsibilities

• Facilitate clients’ ancillary needs and document services provided.
• Ensure clients' TB tests are completed within 7 days of admission.
• Apply clients for state disability and Medi-Cal benefits.
• Arrange transportation for clients to medical facilities.
• Maintain rapport with networking agencies.
